Survey on Meal Preferences

At the request of the Massachusetts Department of Elementary and Secondary Education, Office of Nutrition, Health and Safety, and the John C. Stalker Institute of Food and Nutrition (, we have been asked to encourage our students to participate in a student survey on meal preferences.  This is a 10 minute on-line survey that will help the John C. Stalker Institute in developing menus that are acceptable to all students and also meet the new regulations that have been implemented as part of the Healthy Hunger Free Kids Act.

The same survey/link  (see below) will be used by students in all grades, K-12 and questions will request school district name and grade. Some younger children may need help from an adult to complete the survey.

As a thank you to students for participating, those who choose to provide contact information will be entered into a random drawing; prizes include 10-$25 gift cards and a grand prize of an ipad.

The survey will remain open until Friday, December 21st. and the John Stalker Institute will notify winners of the prizes before January 10, 2013.
